At Open Arms Community Outreach, we specialize in Relocation Service Coordination (RSC-TCM)
Our purpose is to assist residents currently living in nursing facilities with transitioning from institutional settings into independent, community-based living. Our dedicated team partners with each individual to identify safe, affordable housing and arrange the essential services they need to live confidently and comfortably. We prioritize advocacy, dignity, and seamless transitions that promote independence and long-term success. To better serve our diverse community, we also have Spanish-speaking case managers available to ensure that language is never a barrier to support and care.
