What is Relocation Service Coordination?

Relocation Service Coordination (RSC) is a Medical Assistance (MA)-funded program that helps individuals residing in nursing facilities transition into safe, stable housing within the community.  As a specialized form of Targeted Case Management (TCM), RSC focuses on identifying each person’s medical, social, and support needs to create a personalized plan for a successful move.            

At Open Arms Community Outreach, we work in close partnership with individuals, their care teams, counties, and managed care organizations to coordinate essential services and supports tailored to each person’s goals. Our team is committed to ensure that every transition is guided by choice, respect, and long-term independence. We’re here to help make the move from facility to community not just possible—but empowering.          

Who is Eligible?

Individuals may qualify for RSC-TCM services if all of the following criteria are met:


  • Currently eligible for Medical Assistance (MA)
  • Residing in an eligible institutional setting (such as a nursing facility) at the time services are delivered
  • Has made the decision to transition to a community-based setting
  • Consents to receiving RSC-TCM services
  • Has not yet used up their TCM benefit

What Open Arms Community Outreach Can Assist With

Types of Housing

At Open Arms Community Outreach, we support individuals in finding housing that fits their unique needs, preferences, and level of independence. Options include:

  • Independent Apartments – Private residences for individuals who can live with little or no daily support
     
  • Market-Rate or Subsidized Apartments – Affordable housing options based on income, including units where rent is typically capped at 30% of monthly income
     
  • Housing Support (formerly Group Residential Housing) – State-funded housing with supportive services for those with specific needs
     
  • Customized Living Arrangements – Flexible housing options that combine individualized support services with residential living
     
  • Assisted Living – Structured living environments that provide on-site assistance with daily activities
     
  • Adult Foster Care – 24-hour care in licensed, home-based settings offering a family-like environment
     

We are here to help you explore your options and coordinate the right housing solution for a safe and successful transition into the community.

Income Sources

Open Arms Community Outreach assists individuals in applying for and managing a variety of income supports to help ensure financial stability during and after their transition. These may include:

  • Supplemental Security Income (SSI)
     
  • Social Security Disability Insurance (SSDI)
     
  • Retirement, Survivors, and Disability Insurance (RSDI)
     
  • Veterans Benefits
     
  • Pension and Retirement Income
     
  • General Assistance (GA)
     
  • Minnesota Supplemental Aid (MSA)
     
  • Unemployment Benefits (if applicable)
     
  • Other State or County Financial Assistance Programs
     

Our team will work with you to identify all available income resources and guide you through the application and approval process.

Disability Determination Assistance

At Open Arms Community Outreach, we assist individuals in applying for disability benefits to help ensure access to essential services and supports. Our team guides clients through the full application process, including:

  • State Medical Review Team (SMRT) – We help gather and submit the necessary documentation for individuals applying for a state-level disability determination, often required for waiver eligibility or Medical Assistance.
     
  • Social Security Disability Programs – Support with applications for Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI), including guidance on forms, documentation, and follow-up with the Social Security Administration.
     

We strive to simplify this complex process and improve outcomes through personalized, hands-on assistance.

Coordinating Essential Services

We work closely with individuals and their care teams to identify and arrange a wide range of services that support independent, community-based living. Our coordination includes, but is not limited to:

  • PCA (Personal Care Assistance) – Help with activities of daily living such as bathing, dressing, eating, and mobility.
     
  • EW and CADI Waivers – Coordination of Elderly Waiver (EW) and Community Access for Disability Inclusion (CADI) services for ongoing support in the community.
     
  • Independent Living Skills (ILS) – Training to support greater self-sufficiency in areas like budgeting, cooking, cleaning, and using public transportation.
     
  • Supported Employment Services (SES) – Assistance with job development, training, and maintaining employment for individuals with disabilities.
     
  • Nursing Services – Coordination of skilled nursing care, medication management, and health monitoring as needed.
     
  • PT / OT (Physical and Occupational Therapy) – Support in accessing therapies that improve mobility, strength, and daily functioning.
     
  • ARHMS (Adult Rehabilitative Mental Health Services) – Connection to services that support mental health recovery and independent living.
     
  • INR Testing Coordination – Assistance arranging routine blood testing for individuals on anticoagulant therapy.
